Q: Does the holder swivel, or is it in a fixed position?

It swivels. It's a lovely holder. Very pleased.

Q: can you use command strips instead of screws?

If you do decide to use them, I would use the heavy duty command strips. I can't vouch on whether or not that would last long term due to the fact that this item isn't the typical lightweight towel holder. It's got some significant weight to it which is why I used the screws. The other thing you might consider is the fact that the inside of the base where the screws connect is concave so even if you tried to use the command strips, I'm not sure it would catch to the wall based on the recess in the base. I'd stick to the screws it comes with if you can for integrity and long term use.
