Q: On the dt and clk lines, does it need either pull-up resistors and/or debouncing caps?

You need pull ups. Some arduino pins have pull ups but I added 4.7k pull up resistors. Debounce is necessary, I didn't try caps but used a software loop to verify the input level.

It depends on the circuit . This is just a rotary encoder on a pcb with a 0.1" connector.
